Responsibilities:Elwyn is seeking a Team Leader to join the Program for Assertive Community Treatment (PACT). As the Team Leader, you are the team's clinical and administrative supervisor who also functions as a practicing clinician. The team leader, in conjunction with the team prescriber, is responsible for monitoring each client's clinical status and response to treatment and supervising staff delivery of clinical services to maintain a standard of service excellence and courteous, helpful, and respectful services to program clients.
DUTIES AND RESPONSIBILITIES:
- Integrates the Prism Model for effective treatment of serious and persistent mental illness into everyday practice and intervention.
- Provides person-centered, strength-based, trauma-informed care
- Ensures that clients receive effective, understandable, and respectful care that is provided in a manner compatible with client's cultural beliefs and practices and preferred language
- Direct day-to-day clinical operations of the PACT team, including scheduling staff work hours to assure appropriate coverage, leading the daily organizational staff meetings and Community Service Plan (tx. plan) meetings; continuously evaluating the status of clients and doing appropriate planning and coordination of treatment activities to ensure immediate attention to changing needs.
- Coordinates the admission (identifies/addresses any immediate needs or risks) process and treatment, rehabilitation, and support services of the program in coordination with the prescriber. Schedule the intake, develop and coordinate the Comprehensive Assessment and Community Service Plan; assigns the most appropriate staff to the ITT; overall supervision of the Comprehensive Assessment and Community Service Plan development.
- Directs and coordinates the client-centered Comprehensive Assessment of psychiatric history, mental status and diagnosis, physical and dental health, use of substances, education and employment; social development and functioning; activities of daily living; and family structure and relationships
- Introduces the client and the family to the team approach of Comprehensive Assessment and the Community Service Plan
- Utilizes evidence-based assessment tools that support matching a client's needs to their services
- Works with the team to complete a Comprehensive Assessment (within 30 days of admission) as an engagement tool and to understand and respect the client's views of the ways mental illness impacts their life and how they want to be supported in their process of recovery
- In partnership with the client and ITT, develops the Community Service Plan within seven days of the team finalizing the comprehensive assessment.
- Provides clinical supervision according to ELWYN PACT policy, including field supervision
- Provides ongoing assessment of client's symptoms and response to treatment. Make appropriate changes in Community Service Plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior, which puts clients at risk.
- Responsible for monitoring the team's training compliance
- Participates in staff recruitment, interviewing (1:1 and as a team), on-boarding, orientation, and performance supervision.
- Maintains PACT headquarters that are consistent with the PACT model
- Monitors the program budget and utilization of program Contingency funds
- Oversees the EHR record-keeping compliance in accordance with ELWYN and PACT standards.
- Monitors billing and payers, monitor other third-party insurers, and obtain payment from other liable parties for services rendered.
- Consistent with PACT standards, maintain adherence to program policies and procedures.
- Performs on-call duty for crisis intervention and staff supervision and consultation
- Participates in Department of Mental Health contract monitoring meetings
- Along with the team, on an ongoing basis, reviews the outcome measures to evaluate community tenure and independence, submitting the report to DMH quarterly.
- Leads the team to complete the Tool for Measurement of Assertive Community Treatment (TMACT), with ongoing assessment to adhere to the fidelity of the PACT model
- To support the team, provides service coordination for a small group (2-4) clients, including coordination and monitoring the activities of the individual treatment team (ITT); assumes primary responsibility for developing, writing, implementing, evaluating, and revising overall Community Service Plan in collaboration with the client and ITT; provides therapy utilizing evidence-based practices (i.e., CBT-P, CBT, DBT, CAMS, MI), symptom management, ensuring immediate changes are made in the Community Service Plan as clients' needs change; educate and support clients' families, and advocate for clients rights and preferences.
- Provides de-escalation techniques to aid the client (s) in managing a crisis, providing intervention to decrease risk, manage symptoms, and maintain the safety
- Monitors and maintains electronic card-ex and Daily Organization staff assignment schedules
- Leads the team during the daily organizational meeting, offering clinical input and guidance as necessary
- Practices a Housing First approach
- Performs shift management as needed
- Documents client progress, activities, and outreaches in the Electronic Health Record
- Ensures the team is utilizing engagement practices across the adult lifespan.
- Support: provides practical help and support, advocacy, coordination, problem-solving, training, and supervision to help clients obtain the necessities of daily living; assist clients in finding and maintaining safe housing, coaches clients to learn and carry out ADLs, assist and support clients to perform household activities, including house cleaning and laundry, teach money-management skills, nutrition education, meal planning, grocery shopping, access transportation, help clients to have and efficiently use primary care, dentist, and other medical specialist, support clients with structuring their time (community resources, social and leisure activities, hobbies, etc.)
- Awareness of and offers a range of valuable community activities and linkages that support the client's Community Service Plan.
- Performs other duties as assigned
QUALIFICATIONS, EDUCATION AND EXPERIENCE:
Pre-Employment Education/Experience
- Licensed in the State of Delaware as an LCSW, LICSW, or LMHC
- 5+ years' work experience with adults with mental health issues, preferred
- 5+ years' experience in leadership, team lead role
- Preference will be given to candidates that meet the cultural and linguistic needs of the geographic area
Relevant Post-job Offer Testing/Conditions
- Knowledge and practice of ELWYN, DPH, and DMH policies, regulations, and standards
- Must complete PACT Module Training within 21 days of hire
- Participate in other ELWYN required/ recommended training according to the training calendar
- Maintain required clinical license
- Meet or exceed professional development goals
- Demonstrates fidelity to the PACT model
- Ability to make contact and establish relationships with internal and external supports that allow for the program to function within the more extensive system?
- Ongoing education and training in culturally and linguistically appropriate service delivery
- Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record
