Hospice Licensed Vocational Nurse (LVN) - Office Position
Location: Pilot Point, TX

Looking for an LVN role where you can use your clinical knowledge while working in a supportive office environment?

At Concho Hearts Hospice, we believe compassionate care begins with a strong team. We are currently seeking an organized and compassionate Licensed Vocational Nurse (LVN) to join our Pilot Point office team.

This role is ideal for an LVN who enjoys supporting patient care behind the scenes, collaborating with clinical teams, and making a meaningful difference in the lives of patients and families.

What You’ll Do:

• Support daily office and clinical operations.

• Assist with the coordination of patient care and communication with the interdisciplinary team.

• Review and maintain clinical documentation for accuracy and completeness.

• Communicate with patients, families, facilities, and healthcare providers as needed.

• Assist with admissions, physician orders, and other patient care coordination tasks.

• Support nurses and leadership with day-to-day clinical workflow.

• Maintain accurate documentation within the EMR system.

What We’re Looking For:

• Active Texas LVN license.

• Strong communication and organizational skills.

• Ability to multitask and work in a fast-paced environment.

• Experience with EMR systems preferred.

• Hospice, home health, or healthcare office experience preferred (but not required).

• Positive attitude and strong team mindset.
