Halcyon Home is hiring a Home Health Physical Therapist Assistant PRN to support our growing geriatric home health population in South Austin, TX and surrounding areas. We offer competitive pay and wonderful team support.

As a Physical Therapist, you will provide physical therapy to our patients in both community and residential settings in the South Austin area . A minimum of one year of home health experience is preferred, along with a proven ability to manage a fast-paced, independent work schedule. Knowledge of Homecare Homebase software is a plus!
Essential Functions:
- Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and appropriateness of patient for home care.
- Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.
- Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members, when applicable, in the performance of patient care that is consistent with patients’ needs.
- Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
- Accepts clinical assignments that are consistent with education and competence to care for patients.
- Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.
- Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
- Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
- Meets mandatory continuing education requirements of the Agency and licensing board.
- Assists and instructs patients and families in transfer techniques appropriate to patient abilities. Instructs on use of equipment such as walkers, wheelchair, canes, etc., in a safe manner. Utilizes appropriate therapy equipment specific to patients’ diagnoses and needs.
- Provides patient, caregiver, and family counseling; Provides patient and caregiver education.
Compensation based on experience and productivity.
Requirements:- Current PTA license
- Knowledge or experience utilizing Homecare Homebase EMR would be a plus
- Experience in home health or related healthcare setting preferred
- Strong clinical and assessment skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively.
