Case Manager (RN)
Sage Specialty Hospital (LTAC) in Wichita Falls, TX, is thrilled to announce that we are hiring Full-Time Case Manager (RN) for our facility!

Essential duties include:
- Reviews insurance, governmental, accrediting agency and approved hospital healthcare management guideline standards to determine criteria concerning admission, treatment, appropriate level of care, and length of stay for patients.
- Performs utilization review functions of assigned cases.
- Compare inpatient medical records to established criteria and requests interdisciplinary feedback to determine appropriateness of treatment and level of care using pre-established criteria for intensity of service.
- Performs initial assessment for inpatient including: patient/family interviews to obtain full clinical/social knowledge of patient and identifies discharge planning needs.
- Participate in discharge planning and in-service programs related to the needs of the patient
- Great time management skills
Qualifications:
- Current Texas Registered Nurse license
- Bachelor's degree in nursing or healthcare administration preferred
- 3 years of acute care clinical nursing experience preferred
- Minimum of 1 year Case Management experience
- Previous Inpatient Rehabilitation or Skilled Nursing Facility experience preferred
- Good oral and written communication skills
