On-Call Hospice RN - PRN

Job Summary:Primary functions are to administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient, family, and referring agency; and assume the responsibility for coordination of care. Responsibilities:
- Preparing and adjusting patient care plans as needed.
- Providing loved ones and primary caregivers with patient care information, end-of-life education, and emotional support.
- Maintaining and updating medical charts, patient files, and health records.
Job Qualifications:
- Graduate of an accredited nursing program.
- Currently licensed as a registered nurse in the state.
- Two years of experience as an RN in a clinical care setting preferred, hospice preferred.
Skills:
- RN skills as defined as generally accepted standards of practice.
- Good interpersonal skills.
- Specialized skills preferred in palliation and end-of-life. Proof of current CPR.
- Must read, write and comprehend English.
Transportation:
- Reliable transportation.
- Valid and current auto liability insurance.
Environmental and Working Conditions:
- Works in patients' homes in various conditions, possible exposure to blood and bodily fluids and infectious diseases; ability to work a flexible schedule.
- Ability to travel locally.
- Some exposure to unpleasant weather.
- PRN emergency call.
Physical and Mental Effort:
- Prolonged standing and walking are required, with the ability to lift up to 50 lbs and move patients.
- Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition.
- Meet patient/family individualized psycho-social needs.
