Full-Time RN Case Manager - Hospice (Beaumont)

What Your Days Will Look Like:
- Manage a caseload of hospice patients in the Fort Worth area
- Provide compassionate, skilled nursing care in patients’ homes
- Complete visits, admissions, recerts, and documentation
- Collaborate with a supportive interdisciplinary team
- Help patients and families navigate end-of-life care with dignity
- Chart in real time using a provided tablet
What We’re Looking For:
- Texas RN license
- Hospice or home health experience preferred (we will train the right nurse)
- Strong communication and critical thinking skills
- Reliable transportation for local travel
- A nurse with heart, compassion, and independence
