Physical Therapist PT (Home Health) - Full-time

We are seeking a dynamic In-Center Hemodialysis RN to join our team in the heart of Louisville. If you are passionate about providing exceptional patient care and improving the quality of life for individuals undergoing dialysis, we would love to hear from you!
Key Responsibilities:
- Administer hemodialysis treatments to patients in accordance with prescribed protocols.
- Monitor patient responses to treatment, identifying and addressing any complications or adverse reactions.
- Develop and implement individualized patient care plans in collaboration with other healthcare professionals.
- Educate patients and their families about dialysis procedures, treatment plans, and self-care strategies.
- Maintain accurate and detailed patient records, ensuring compliance with healthcare regulations and standards.
- Participate in continuous quality improvement initiatives to enhance patient care and outcomes.
Qualifications:
- Graduate from accredited nursing school (BSN preferred)
- Current state RN license
- 1 year nursing experience
- Hemodialysis training completed
- Current CPR certification (AHA)
- Valid driver’s license and reliable transportation
